Purpose. This form is used to communicate with Village personnel to request services and information.
Note: Route 45, 83, 120 & 137 fall under the jurisdiction of the Illinois Department of Transportation (IDOT) and Center Street (east of Route 83) & Washington Street fall under the jurisdiction of the Lake County Division of Transportation (LCDOT). If your request falls under the jurisdiction of another agency, then the Village will forward your request to the appropriate agency for action. We will mark the request as "Resolved" when we have done so.
