This application must be filled out if you are planning a special event in the City that will require a street closure or is being planned at a City park (this does not have to be filled out for basic shelter rentals).

* Will there be alcohol served separate from the Designated Outdoor Refreshment Area (DORA)? If so, who will be obtaining the Special Event Liquor License?
* Will food be served? If yes, you must contact the Clermont County Health District for inspection information.
* Will tents/stages/booths or any other temporary structure be used? (tents over 200 sq ft require a permit)
