Police Report Request

Purpose. This form is used to communicate with the Miami Township Police Department to request a copy of a police report of an incident that occurred within the limits of Miami Township. Upon the successful entry of the necessary information, the report copy will be returned to you in one of three methods which you may choose: U.S. mail, fax or pick-up at Police Headquarters at 5900 McPicken Drive. After placing your order, you will receive an email confirmation of the request and a later follow-up notifying you when the report will be sent or is ready to pick-up. Questions?
