Instructions: This application should be completed by the property owner or project contractor and must be accompanied by a completed UCC Application. The UCC Application must be completed by a PA Certified Design Professional. The completed forms are to be submitted to the Department of Community Services Building Inspection Office for review/approval. Submission may be online or in person.
