Park Shelter Reservation Request

*** RESERVATIONS FOR PARK SHELTERS CAN BE MADE NO MORE THAN 6 MONTHS
PRIOR TO YOUR DESIRED DATE ***

Use this form to apply for a reservation of one of the park shelters in the Miami Township Park system. Please note that your reservation is not guaranteed until you receive a follow-up email saying the reservation is CONFIRMED.

FEES: Fees are $50 for Miami Township residents and $100 for non-Miami Township residents for all shelters except the Community Park Performance Pavilion ($25 Resident - $50 Non-Resident).

Non-Profit groups with a 501C3 designation may reserve the park pavilions, on a first come first serve basis, any Thursday of the calendar year - at no charge.

Reservations are for rain or shine and the fee is refundable if the event is cancelled 30 days in advance of the scheduled date. Reservations must be made by a Township resident or business and by an adult at least 18 years of age who assume responsibility for the group and condition of the area after the group has departed. The reservation can be made at MiamiTwpOH.gov but payment must be made in person or by mail at the Miami Township Recreation Department at 6101 Meijer Drive WITHIN FIVE DAYS OF THE CONFIRMED RESERVATION DATE or the reservation will be cancelled.

GENERAL RULES & REGULATIONS:

1. The consumption of alcoholic beverages is permitted at any park shelter with an approved permit (PLEASE READ THE ALCOHOL RULES BELOW).

2. All litter must be properly disposed of and the general appearance of the shelter must be in the same condition as before the event.

3. notice of reservation must be posted at the shelter by Applicant at least 24-hours in advance of the start of the event. Applicant must also keep a copy of the confirmed reservation form on person during the event.

4. All other park rules and regulations are to be observed.

5. Some park shelters are near concession, restroom and playfield areas. The township cannot guarantee that noise and pedestrian traffic will not interfere with the activities occurring in the shelter. Hours of Operation: Park Pavilions are available for rental from 10 a.m. to Dusk.

In conjunction with my application for use of a Park Shelter, I may request permission to serve alcohol at my event. I further understand that I must provide adult supervision to prevent consumption of alcoholic beverages by minors. I will not charge or solicit donations for admission to the event and/or for the alcoholic beverages. I agree to indemnify, hold harmless and defend the township, its officers, employees, and agents against any and all liability, loss, costs, damages, expenses, claims or actions, including attorney's fees which the township, its officers, employees and agents may hereafter sustain, incur or be required to pay, arising wholly or in part due to any act or omission of its agents, servants or employees, in the execution, performance or failure to perform my obligations pursuant to the terms and conditions of this permit. I affirm that I have read and understand the terms and conditions of the Miami Township Alcohol Permit Policy. This permit must be kept with the permit holder during the duration of the scheduled event and is subject to inspection by any employee of Miami Township.

Miami Township Alcohol Permit Policy

1.0 Purpose: 1.1 The purpose of this policy is to establish the procedures for obtaining permission to consume any form of alcoholic beverage on township-owned property.

2.0 Scope and Definitions:

2.1 This policy applies to the issuance of alcohol permits at the Miami Township Civic Center, Leming House and all township park pavilions.

2.2 The Miami Township Administrator or designee has authority to grant permission to persons to consume alcoholic beverages on township-owned property.

2.3 For Civic Center permits, no alcohol can be consumed outside the room that is rented. For Leming House permits, no alcohol can be consumed outside the building with the exception of the exterior deck. For park shelter permits, no alcohol can be consumed 30' beyond the perimeter of the shelter.

3.0 Policy

3.1 Permit Holder. The permit holder must be an individual of at least 21 years of age.

3.2 Rules of Conduct: The permit holder shall be responsible for maintaining order at the reserved facility both inside any building and on the grounds. The permit holder shall remain on the premises at all times; Persons using the facilities are responsible for complying with State, County, and Township regulations, including Miami Township Park Bylaws and Facility Rental Rules and Regulations; There will be no loud noise or inappropriate conduct that would infringe on the reasonable privacy of individuals in the facilities; The permit holder shall be responsible for ensuring that the activities conducted in the facility are contained within the area which is reserved; Glass containers for alcoholic beverages are not allowed in any of the permitted areas; The reserved facility shall be left in a state of cleanliness and good repair; The sale of alcoholic beverages at reserved facilities is prohibited; The permit holder shall be responsible for ensuring that alcoholic beverages are not served to minors. The Miami Township Police Department may conduct periodic checks of reserved facilities to ensure compliance with state statutes governing the consumption of alcohol.

3.3 Miami Township retains the right to revoke, modify, or cancel the policy at any time and as it sees appropriate.

3.4 This policy shall become effective upon its adoption.
