Purpose. This form is used to communicate with city personnel to request services and request information. A permit is required for any work done within the right-of-way on any public street. Permits can be obtained at this office or downloaded from the documents tab. Permits are $3.00 and need to be submitted with a brief sketch including dimensions of proposed work and contractor information. Please allow 48 hours for processing.
